LHI Is the medical coordinator for Army Reservists. When you are told you need services from dental to PHAs to vision screenings they are scheduled through them.
Medical Readiness is one of the biggest priorities in the Army right now across all components. You may wonder why PHAs and dental appointments are such a big deal. It’s because they can reveal health problems or teeth problems that take time to fix. If you got a mobilization order tomorrow and need treatment after treatment for a dental issues you would not be able to go given issues like that can turn into major infections in an austere environment.
You must get a dental exam every 12 months and a PHA every 15. However, you can schedule them 90 days before they are due and we highly recommend that so you never go “red”.
If you go red you are only allowed to sign in for the morning of the first day of Battle Assembly then will be sent home with Us for the remainder. Keep in mind you can only get 9 Us in 12 months before becoming an UNSAT and a separation process can begin.
You will get paid a four hour block to attend your mandatory appointments. After your appointment you should have a signed voucher from the provider you went to. If you download an app like Microsoft Lens or Adobe Scan on your phone can you scan your voucher as a PDF and upload it below to get paid even faster.
Steps to scheduling a PHA:

- Have the following information ready:
- Your Branch: Army Reserve
- Our UIC is WS5UAA
- Drilling Status: TPU (Drilling Monthly)
- Your Social Security Number
- Your Full Name and Rank
- Have a calendar in front of you to see when you are available and to write down appointments
IMPORTANT: A PHA requires you to fill out an online questionnaire prior to getting scheduled for an appointment. They may tell you to go to https://www.lhi.care to take it. Do this as soon as possible or else you won’t get an appointment. At the end of the online survey you MUST call the number they provide so a live person can review the results with you and ask you several questions. Failing to complete all these steps will prevent you from getting an appointment.
- Continue to call LHI if you do not hear back from them. They can be slow to schedule appointments.
Option 2: Go to https://www.lhi.care
- If it is your first time there you’ll need to register for an account. You don’t need to use your .mil email address.
- Once you have an account you can log in and have them check if any services are due. You MUST hit the “Request Services” for them to check. “No Upcoming Appointments” does not mean you don’t have any upcoming services. See below.
- After you request services you will get an automated voicemail from 877-437-6313 (save this number in your phone as LHI, so you don’t miss their calls). They will tell you they are working on scheduling your services. Go back online and you’ll likely see you can complete then online portion of the PHA.

- Bring this form with you printed out and your information filled out on top. The dentist can fill out the rest.
- Take the completed form and upload it on https://www.lhi.care where it says “Request Records Update”. Don’t upload it to our tool below. That is only for vouchers. Bring a copy to drill and we can pay you for the visit to your dentist.
